LONG INFO:

How to Fill Out the Info Section:

Extra items worn - accessories, stuff like necklaces and collars (if not shown on the drawing).
Notable features - glowing eyes, split tongues, etc which can't be seen on the drawing.

Age - see this for reference x.
Attributes - see below.
Goal - the one thing your character wants to accomplish above everything else.
Personality - make sure to add some negative traits too! See this for reference x.

Job - occupation, social status, eg. kit, apprentice, warrior, deputy.
Current/Previous Affiliation - with whom your character worked/was aligned with, eg. ThunderClan, BloodClan.
Rank - same as job.

Attributes

This is an attribute map, otherwise known as a skill graph. There are 10 attributes, connected by lines to the centre. These lines are divided into 5, and each point is a level. 
E.g. if your character's strength level is 4, you put a mark on the fourth point from the centre. If your character's intelligence is level 3, mark that, and connect the two marks. Repeat for the entire graph, and you should get a polygon.

Strength - physical strength, plays a part in attack and defense.
Intelligence - how quickly your character can figure out and apply knowledge. 
Knowledge - how many things your character knows.
Charisma - how good your character is with others, or how well they can convince others.
Agility - reflexes, dexterity, plays a part in attack accuracy and evasion.
Stamina - physical endurance, including to poisons and exertion. 
Perception - how aware your character is of their surroundings, and how quickly they notice something.
Wisdom - common sense, or spirituality, important for medicine cats.
Attack - how well your character can attack/deal damage.
Defense - how well your character can defend/prevent damage.

How Edit It

- Open in any drawing program. (Please note it was made with MS Paint in mind, but also works in Photoshop, Gimp, Sai and any program with a selection tool)
- Click the selection tool. (Can also be called the Magic Wand/Lasso tool. Anything that allows you to select something and move it)
- Select a feature from the Add-Ons bar.
- Drag onto the main model.
- Flip/Copy+Paste if necessary.
- Erase any stray lines.
- Ta-daaa!

General Rules of Use

- Any and all kinds of editing are allowed as long as you keep my watermark.
- You can do basically anything as long as you keep my watermark somewhere.
- Any and all groups, roleplayers and others can use this in their roleplays and groups as a reference/application sheet.
